
Tom Kolacki, PT
Owner
Tom has been a practicing physical therapist since graduating from the University of Illinois in 1988. It did not take long after his graduation that he realized his goal of owning his own private practice physical therapy clinic. In 1990, he joined forces with George and opened Southwest Physical Therapy. Whether in homecare or the outpatient clinic, Tom treats his patients with compassion and understanding while pushing them to achieve the goals set between him and his patient. He wears many hats, but never sacrifices his patient's treatment, taking a hands-on approach with all his patients. He enjoys working with individuals who are seeking physical therapy as an option instead of surgery but also excels at post-op recover. Most recently he has become certified in the Graston Technique. He stays current with all legislative and pertinent trends in private practice physical therapy and was integral in the Direct Access legislation allowing physical therapists to treat without a doctor's script. Tom is a huge Chicago Blackhawks and Chicago Bears fan and enjoys camping and traveling to Colorado to visit his two children.

George Cachares, PT, MS
Certified Vestibular Therapist, Clinic Manager
George is a 1981 graduate of Physical Therapy from Northwestern University. He has earned a post-graduate degree in Rehabilitation Management from DePaul University. He has worked side by side with Tom, since 1990, treating a wide range of clients and their physical therapy needs. His specialties vary from orthopedics to chronic pain. He has become well recognized as one of the top vestibular therapists in Chicagoland helping numerous patients with balance issues and vertigo. George supports the efforts of independent practice focusing on successful outcomes, satisfied clients in a friendly, happy clinic. George remains committed to supporting the success of community-based physical therapy and independent small business ownership. He is very active within the Oak Lawn Chamber of Commerce. George is married, has three children and loves to travel - particularly to the mountains to downhill ski.

Jessica Rolla, DPT
Physical Therapist
Jessica graduated with her doctorate in physical therapy from Governors State University in 2011. She has interest in applying her knowledge in orthopedic management and continues to use her talents directed toward clinically effective functional exercise. She has an interest in working with fall prevention, joint replacement clients and acute post-injury athletes. She is proud of her success in her career and has many success stories among the patients who have benefited from her treatment. She loves enjoying her free time with her husband and two daughters.

Mauricio Diaz
Client Care Specialist and Clinic Aide
Mauricio came to us as a student of St. Xavier University and has since obtained his Bachelor of Science in Health. Mauricio is dedicated to managing the front desk along with assisting the therapists with their patients. His goal is to become a Doctor of Physical Therapy. In his spare time, when not studying, he enjoys sports and music.

Lori Jatho
Insurance and Billing Administrator
Lori has been an integral member of Southwest Physical Therapy/FYZICAL for over 20 years. She prides herself on providing excellent employee and customer relations. In 2017, Lori took over the title of Billing Administrator for FYZICAL. For any and all insurance and billing questions Lori is happy to assist. Making certain that clients are getting what they need from their insurance provider is a daily motivation. She will also gladly take a stroll through the clinic to discuss with patients the latest episode of “This Is Us”.

Mona Kolacki
Client Care Specialist, Director of Marketing
Mona has been an integral part of the daily operations of Southwest Physical Therapy Home Care for 15 years. She is effective in her role as Client Care Specialist and TLS Weight Loss Coach. Mona is a graduate of University of Illinois Urbana-Champaign with a degree in Nutrition and Dietetics. With her degree and passion for fitness and healthy eating, she has become our clinic's weight loss coach. She is married and has two children living in Colorado, loves running, biking and horseback riding.
